Ankle fractures and ankle ligament injury

A stable fracture can often be treated in a cast or boot with follow-up X-rays. An unstable fracture, or one where the joint surface has moved, usually needs an operation to realign the bones and hold them with plates and screws, timed for when swelling has settled if the skin is very swollen.

Achilles tendon rupture

A sudden snap at the back of the heel during push-off can mean a ruptured Achilles tendon, with a weak calf and no rising on the toes of that leg. Treatment is either a boot with early guided movement or a repair operation, chosen after examination, as described on the Achilles tendon surgery page.

Heel pain and plantar fasciitis

Heel pain that is worst with the first steps in the morning usually comes from the plantar fascia, the band under the sole. Treatment is calf and sole stretching, cushioned footwear and physiotherapy, and most cases improve over several months. Shockwave therapy is offered when the pain has not settled, and surgery is considered only after about 6 to 12 months of proper non-surgical care.

Bunion and flat foot

A bunion (hallux valgus) is a bony bump at the base of the big toe, with the toe drifting towards the others. Wider footwear, toe spacers and padding relieve symptoms but do not straighten the toe. Surgery is considered when pain continues and limits walking or footwear. In adults, a flat foot that develops later, with pain along the inner ankle and a heel that tilts outwards, can come from a worn posterior tibial tendon. Diabetic foot

Reduced feeling in a diabetic foot lets small injuries go unnoticed and turn into ulcers. Orthopaedic care looks at foot shape, pressure areas, protective footwear, and surgery for deformity or infected bone where needed, together with the diabetes physician. Clubfoot in children

In clubfoot, one or both feet turn inwards and downwards from birth. Treatment usually starts in the first weeks of life with gentle casts by the Ponseti method, a small heel-cord release in the operating theatre in many babies, and a brace worn for several years. See the clubfoot page.

Ankle arthroscopy and ankle fusion

Arthroscopy looks into the ankle through small cuts, for pain, catching or stiffness after injury when scans point to a problem inside the joint, such as bony spurs, loose fragments or cartilage damage. For advanced ankle arthritis, ankle fusion is offered. Ankle replacement also exists but is not done by Dr. Khanna, and patients who may suit it are referred. Preparing for surgery, recovery and risks in general terms

Before foot or ankle surgery there are blood tests, an anaesthesia check and a review of medicines. Smoking slows wound healing, and diabetes needs to be well controlled. Afterwards the leg is kept raised in a splint or boot, with crutches. Recovery depends on the operation and is explained beforehand. Risks include wound problems, infection, stiffness, nerve irritation, a blood clot in the leg, and pain or deformity that does not settle completely.
